Service fee includes:
- Thorough and individual assessment encompassing your complete medical history, coupled with an exploration of your personal goals.
- ​In-depth blood work and hormone level investigation
- An individualized treatment plan based off the assessment and your needs
- The initial consultation includes a 2 week follow-up to review  blood, prescription if indicated and support on how to safely use your medication. If more time is required, a 30 minute follow-up appointment is recommended.
- Evidence based diet and exercise recommendation 
- Following this initial consultation and 4 week follow-up is recommended

Medical Weight Management - Initial

Service fee includes:
- Completed at 4 weeks
- Refills if necessary 
- Discuss your progress 
- Provides an opportunity to monitor your progress closely and, if necessary, make any adjustments to your personalized plan.
- Basic diet and lifestyle resources 
- Establish a plan for subsequent follow-ups

Medical Weight Management - Follow-up

Service fee includes:
A comprehensive 60 minute assessment of your health concerns, goals, and medical history, allowing us to understand your current situation thoroughly. This process helps us pinpoint any underlying issues and identify any gaps in information.
Cycle and symptom tracking.
A review your current diet, lifestyle, current stressors.
Thorough blood work requisition based on your individualized needs.
Review recent blood work or diagnostic testing (see FAQs for more information).
A personalized Q&A session to address all your questions and concerns from an holistic lens.
Goal setting.

PCOS - Phase 1 - Initial Consultation

Follow up 2-4 weeks after initial visit:
Service fee includes: 
- In depth review of blood work. 
- Determine the need for additional diagnostics based off blood work.
- Review cycle and symptoms.
- Discuss a management plan including supplements and/or prescriptions. 
- Evidence based basic diet and lifestyle recommendations with goal setting.
- If no period occurred, discuss treatment options. 
- Discussion of next steps.

PCOS - Phase 1 Follow-Up

This phase is optional. 
Further investigation of issues include:
- Rashes and skin issues (i.e acne, post inflammatory scarring, etc)
- Ongoing thyroid dysfunction 
- Residual inflammation
- Liver and gallbladder concerns including fatty liver, gallbladder pain
* some of these listed concerns may have been resolved, thereby skipping this phase is an optional if determined mutually between you and your practitioner *

PCOS - Phase 2 - Persistent Symtoms

Service fee includes:
- In depth blood work where you will achieve a great understanding
- Evidence based basic diet and lifestyle recommendations
- Medication dosing if required

PCOS - Phase 2 - Follow-Up

30 minute follow-up.
- Review of current symptoms.
- Review of cycle tracking.
- If still no period occurred, discuss next steps.
- Re-test labs if indicated to monitor improvements. 
- We'll keep an eye on your progress and make adjustments to your treatment plan if we're not seeing the results we hoped for.  In this phase, you can touch base with your NP. No judgment.

PCOS - Phase 3 - Maintenance

Our preventive medicine service is dedicated to safeguarding the health and well-being of both individuals and defined populations. It focuses on promoting and maintaining optimal health, as well as preventing disease, disability, and premature death


Service Fee Includes:
- Comprehensive history including medical history, evaluating lifestyle factors, social habits, stress levels, environmental influences
- Discussion about lifestyle and recommendations for psychological well-being.
- Evaluation of fitness and diet history
- Evaluation of current medication and supplements history
- Current goals and interests
- Requisition for personalized blood work

Preventative Health

- Deep analysis of blood work relative to your lifestyle
- Review of risk of metabolic condition 
- Treatment plan and recommendations  to maintain/ potentially reverse/ treat
